As an Evolve Med Spa VIP member, your payments will be collected monthly. Complete payment for products and services rendered must be made at the time of treatment. Membership discounts cannot be combined with other offers or discounts.

All products and services offered at Evolve Med Spa are subject to change. Members will be notified of any changes to Membership pricing, terms, or conditions at least 15 days in advance.
You may cancel your Membership at any time after 3 months.
Memberships cannot be cancelled within the first 3 months. Line tamer Memberships are for quarterly treatments and you must complete the quarterly payments per quarterly treatments before canceling.
Please note that we do not pause Memberships.
Memberships at Evolve Med Spa are non-transferable and must be redeemed by the member only.
You must be 18 years of age or older to sign up for a Membership.
For Memberships including complimentary services, you have 30 days from the renewal date to redeem your service. If not redeemed, this service will expire and will not roll over.
Any credits must be used before canceling, or they will expire on the last day of your Membership period.
